Paladin Summer Hockey Camp Disclosure Agreement

This agreement ("Agreement") is entered into on June 3rd, 2024 by and between Paladin Summer Academy hereinafter referred to as the "Camp Organizer," and the undersigned participant or parent/guardian of the participant, hereinafter referred to as the "Participant."​

Disclosure Agreement:
 

1. Health and Medical Conditions:

   • The Participant or their legal guardian hereby acknowledges and agrees to disclose any pre-existing medical conditions, allergies, or other health concerns that may affect the Participant's participation in the Paladin Summer Hockey Academy.  Paladin Summer Hockey Academy shall not be held liable for any medical emergencies or complications arising from undisclosed conditions.
      

2. Assumption of Risk:

   • The Participant or their legal guardian acknowledges that participation in the  Paladin Summer Hockey Academy involves inherent risks, including but not limited to physical injury, illness, or property damage. The Participant voluntarily assumes all such risks and agrees to release the Paladin Summer Hockey Academy from any liability for injuries or damages sustained during the camp.
      

3. Insurance:

   • The Participant or their legal guardian understands that it is their responsibility to ensure that the Participant has adequate medical insurance coverage for the duration of the summer hockey camp. The Paladin Summer Hockey Academy does not provide insurance coverage for participants.
      

4. Behavior and Conduct:

   • The Participant agrees to conduct themselves in a manner that is respectful to coaches, staff, fellow participants, and facilities. Any behavior deemed inappropriate, including but not limited to bullying, harassment, or substance abuse, may result in immediate dismissal from the Paladin Summer Hockey Academy without refund.
      

5. Photography and Media Release:

   • The Participant or their legal guardian consents to the  Paladin Summer Hockey Academy use of photographs, videos, or other media captured during the summer hockey camp for promotional and marketing purposes, including but not limited to website content, social media, and printed materials.
      

6. Parent/Guardian Consent:

   • In the case of a minor participant, the undersigned parent or legal guardian hereby consents to the Participant's participation in the Paladin Summer Hockey Academy and agrees to be bound by the terms of this Agreement on behalf of the Participant.
      

7. Acknowledgment of Understanding:

   • The Participant or their legal guardian acknowledges that they have read and fully understand the terms and conditions of this Agreement, and they voluntarily agree to be bound by its provisions.
